About the Role
As a Support Worker with ChoiceOne, you’ll provide high-quality, person-centred care to clients in community and disability settings. This role involves supporting clients with both daily living activities and complex care needs , while maintaining dignity, safety, and respect at all times.
Shifts are available across a range of Perth metro locations, offering flexibility and opportunities for ongoing placements.
Key Responsibilities
- Personal care (showering, dressing, hygiene support)
- Hoisting and safe manual handling (sling hoists, slide sheets, transfers)
- Medication administration
- PEG feeding and complex bowel care
- Catheter and stoma care
- Tracheostomy care
- Seizure management and epilepsy support
- Diabetes and asthma awareness
- Meal preparation, cooking, cleaning, shopping, and household support
- Accurate progress notes and record keeping
- Shift handovers and communication with incoming supports
- Other duties in line with individual client care plans
Minimum Requirements
- Certificate III in Individual Support (or relevant equivalent)
- Minimum 12 months’ experience as a Support Worker
- NDIS Worker Screening Check
- Working With Children Check (or willingness to obtain)
- National Police Clearance (issued within the last 12 months or willing to obtain)
- Current Driver’s Licence with own vehicle
- Medication & PEG competency certification
- Epilepsy awareness and seizure management (including medication)
- Experience supporting challenging behaviours
- Current Manual Handling certification
- Experience driving wheelchair-accessible vehicles (desirable)
